Thank you for your interest in completing a medical education rotation at Evara Health (Evara). Your assistance in completion of the following form will help determine the best type of placement for your rotation. A representative will contact you to discuss rotation placement options. Please note that the maximum duration of a medical education rotation at Evara is 8 weeks. If your desired dates of rotation extend beyond 8 weeks, you will need to re-apply for an extension. Rotation applications are accepted no greater than 90 days prior to the rotation start date.
