Payment Options
We accept Medicaid, Medicare, Healthy Kids/Florida KidCare and most private insurances. Any remaining balance will be the responsibility of the patient.
A Sliding Fee Discount Program is available for all eligible patients. Proof of income must be presented to qualify.
Florida KidCare, ACCESS Florida and Health Insurance Marketplace enrollment assistance available through one of our Community Services Specialists.
Payment options include pre and prompt pay discounts. Please call 727-824-8181 and ask for details.
Good Faith Estimates
You may have the right to receive a “Good Faith Estimate” explaining how much your health care will cost.
If you do not have insurance or you are choosing not to use insurance, you have the right to receive a Good Faith Estimate of the expected cost of your scheduled healthcare services. If your appointment is scheduled at least 3 business days in advance, Evara Health will provide a written Good Faith Estimate within the timeframe required by law. You may also request an estimate before scheduling care.
If your final bill is at least $400 more than your Good Faith Estimate, you have the right to dispute the bill through the federal patient-provider dispute resolution process.
