Evara Health’s Women’s Health is accredited by the National Committee for Quality Assurance (NCQA) and by the Accreditation Association for Ambulatory Health Care (AAAHC). Evara Health is Certified by AAAHC as a Patient-Centered Medical Home: the model is best described as a philosophy of medical care that is patient-centered, comprehensive, team based, coordinated, and focused on quality and safety.   

Evara Health provides individualized care for all women through all stages of life. That’s the philosophy of our Women’s Health team. With a dedicated and committed team of board-certified medical providers and advanced practice clinicians (which may include nurse practitioners and certified nurse midwives), we focus on a wide range of women’s health issues, including routine care, diagnostics, reproductive health, family planning, as well as complete obstetrical care: prenatal, labor and delivery, and postpartum. Our Women’s Health team works with a multi-disciplinary team of professional, paraprofessionals and lay workers, including RN’s, LPN’s, care coordinators, pharmacists, and behavioral health therapists.  

​We also combine the comfortable environment of a small family practice with the modern amenities and flexibility you’d expect from a large group practice. And because women’s health is so very personal, our providers put you in control of your health and offer care that aligns with your values.

Maternal Health

Evara Health is here to provide personalized care that prioritizes both you and your baby’s health.

Embarking on the journey of motherhood is a transformative experience, and at Evara Health, we are dedicated to providing comprehensive Maternal Health Services that prioritize the well-being of both mothers and their precious little ones. From the early stages of family planning to postpartum care, our range of programs are designed to support and nurture every aspect of the maternal journey. A key component is care coordination, and along with our Maternal Health programs we offer our pregnant patients access to additional Evara services – Nutrition, Behavioral Health, Dental.

Maternal Health Programs at Evara Health

Remote Patient Monitoring: Experience the convenience of Remote Patient Monitoring at Evara Health, where we empower patients with medical equipment to monitor vital signs such as blood pressure and blood glucose in the comfort of their homes. These vitals seamlessly link to a monitored portal, staffed by our dedicated team from 8:00 AM to 5:00 PM on weekdays. With this innovative approach, our providers can intervene promptly if needed, ensuring continuous care and peace of mind for both mom and baby.

Medical Home at Home: Evara Health brings healthcare to your doorstep with our Medical Home at Home program. Our trained medical assistants, equipped with a mobile clinic, visit homes at key milestones—3-5 days and 2 weeks of a baby’s life. Conducting vitals and coordinating telehealth visits with pediatric and women’s health providers, our team ensures personalized care for both mom and baby. Additionally, we offer point-of-care testing, including urine culture, hematocrit, bilirubin, and blood sugar, right in the comfort of your home.

Food Rx Pharmacy: Evara Health believes that healthy food is medicine and offers access to fresh and healthy fruits, vegetables, and staple foods through the Food Rx Pharmacy program in collaboration with Feeding Tampa Bay.

To be eligible for participation in this program, patients must:

  • Answer the food-related questions during Women’s Health appointment registration
  • Have a qualifying health condition
  • Be referred by a Women’s Health provider

Participants in this program receive a variety of fresh produce and nutritious non-perishable food at no cost. Once enrolled in the Food Rx program, participants receive vouchers to pick up food using the method and location that is most convenient for them. Participants can pick up their groceries at one of Evara Health’s on-site pantries or Feeding Tampa Bay’s Groceries On The Go (GOTG) mobile grocery store.

Staff Training and Community Outreach: Evara Health is committed to excellence not only in patient care but also in empowering our staff with specialized training. Our Primary Care Providers (PCPs) undergo comprehensive training in preconception counseling, promoting healthy pregnancy behaviors for all women of childbearing age. Additionally, all our team members receive training in trauma-informed care, adverse childhood experiences, and cultural competency. We go beyond by extending this knowledge to the community through educational classes, fostering a supportive environment for all.
