Evara Health’s Primary Care is accredited by the National Committee for Quality Assurance (NCQA) and by the Accreditation Association for Ambulatory Health Care (AAAHC). Evara Health is Certified by AAAHC as a Patient-Centered Medical Home: the model is best described as a philosophy of medical care that is patient-centered, comprehensive, team based, coordinated, and focused on quality and safety.
Evara Health’s Primary Care services are provided by board-certified physicians and advanced practice clinicians (which may include nurse practitioners and physician assistants). These primary care providers address the main health issues in our community such as hypertension, diabetes, and asthma, among many other chronic diseases. Primary care providers also offer family planning services.
Our primary care providers coordinate the provision of preventive and acute care via a multi-disciplinary team of professional, paraprofessionals and lay workers, including RN’s, LPN’s, care coordinators, pharmacists, and behavioral health therapists. Care is coordinated and encourages patient engagement through all aspects of the health care spectrum.
