Juvenile Welfare Board – Children’s Mental Health Initiative (CMHI). This program is designed to enhance public awareness of children’s mental health issues and provide integrated behavioral health services within primary care pediatric settings.

AHCA/NSU – Family Medicine Residency Program. The mission of this program is to train, educate and develop caring, mature, professionally competent Family Medicine physicians. In partnership with AHCA and Nova SE University, Evara Health aims to train, and retain, physicians who are sensitive to the needs of the medically underserved in our communities.

Florida Department of Education – In 2023 the Evara Health Institute applied for nearly $300K in funding to enable Evara to offer the region’s only Medical Assistant Registered Apprenticeship Program, developed to mitigate critical, community-wide shortages of medical assistants. This partner-ship will also provide upskilling opportunities for Evara employees who seek to advance in their healthcare career.

Pinellas County Government and Pinellas Community Foundation – Grant awards from these partners provided funds for operations and equipment to support expansion of Evara Health’s Largo facility to give patients access to much-needed dental services in an underserved area.

Health Resources & Services Administration – Maternal Health. In 2023 Evara Health
was awarded a $2 million grant to address racial disparities in maternal and fetal mortality and morbidity rates. The funding is used to invest in specialized prenatal care programs that offer tailored support and education to mothers. Our Maternal and Infant Health Program champions innovative approaches using remote patient monitoring devices and in-home clinical team visits to improve patient engagement, enhance pregnancy monitoring, and increase early intervention when problems are identified.

BayCare – Services to End Recidivism (STEER). The STEER program is a collaborative effort, funded by BayCare, to provide primary care services via Evara’s Mobile Health Center, to enhance the continuity of care and to reduce the emergency department utilization by those who receive services at St. Vincent de Paul and WestCare/Turning Point.

American Cancer Society & Robert Wood Johnson Foundation – Health Equity Project. This collaborative provides an opportunity for Evara Health, the American Cancer Society, and the Robert Wood Johnson Foundation to strengthen their relationships with each other in addressing obstacles preventing people of color from accessing colorectal cancer screening and follow-up care. The collaboration tackles medical mistrust, and it identifies community-driven solutions to influence practice and policy changes aimed to increase screening and reduce cancer rates.

Pinellas Heights Housing Authority – Utilizing Evara Health’s dental mobile health center, we are able to increase dental access for our patients through this partnership. Residents can receive comprehensive dental services, including exams, x-rays, fillings and routine extractions. Residents also have the option to be seen at one of Evara’s convenient brick-and-mortar locations.

Health Choice Community Partners, LLC. – (HCCP) was formed as an accountable care organization (ACO) in 2022 with the mission of improving the health status of our communities through the provision of affordable, evidenced based and community-based care. HCCP aims to promote and support quality medical homes through the re-investment of savings back into the community and through the meaningful use of health information technology and data analytics. 

HCCP was formed by community health centers and Health Choice Care (HCC) to develop a provider-sponsored, community health center-based organization capable of providing quality care to our communities while adapting to changes in the health care environment. HCCP focuses on achieving the quadruple aim of improving the health of populations, improving the clinician and patients experience of care, and reducing per capita health care costs.

HCCP is jointly owned by seven community health centers located throughout Arkansas, Florida, Kentucky, Mississippi, Illinois, Texas, Rhode Island, and Health Choice Care (HCC).
