Do I have to be an Evara patient?
Many of our Community Services are designed to support Evara Health patients, but we're always happy to talk with you about your needs. If you're not yet an Evara patient, we'll help you understand what's available and how to get connected with our care team.
Is there a cost for Community Services?
Not at all! There is no charge to speak with one of our Community Services Specialists. They're here to connect you with programs and resources that can support your health and wellbeing.
Can you help me apply for Medicaid?
Yes! Our team can help you understand your options and guide you through the application process for Medicaid, Medicaid for Pregnant Women, Florida KidCare, Marketplace plans, Medicare Savings Programs, and other available coverage programs.
Can you help me renew my health insurance?
We sure can. If it's time to renew your Medicaid or other health coverage, our Community Services Specialists can help you complete the renewal process and answer your questions to help prevent a lapse in coverage.
What if I don't qualify for a program?
We'll help you explore other options. Every situation is different, and our team can connect you with additional community resources and support programs that may meet your needs.

How do I meet with a Community Services Specialist?
Getting started is easy. Ask your provider for a referral, speak with a member of your care team during your visit, or call 727-824-8181 to be connected with a Community Services Specialist.

Absolutely! You don't need to know which program you're looking for before reaching out. Tell us about your situation, and we'll help connect you with the resources that may be right for you.
What kinds of things can Community Services help with?
Our team can help with health insurance enrollment, transportation to medical appointments, food assistance, financial assistance programs, Medicare Savings Programs, patient portal support, early childhood education resources, and more. If we can't help directly, we'll do our best to connect you with someone who can.
