Family Medicine (Adults)
If you're looking for an appointment for yourself (an adult patient) or another adult 18 years or older, Family Medicine may be the best fit for you.
If you have NOT been seen at Evara in three years, please choose the New Patient Appointment option.
Pediatrics (Kids)
If you're looking for an appointment for a child or children from birth to age 20 years, Pediatrics would be the best option.
If you have not been seen at an Evara location in the past three years, please choose the New Patient Appointment option.
TeleMedicine
If you're looking for a Telemedicine appointment for yourself or another adult over the age of 18, Telemedicine may be the best fit for you.
Evara Gold
If you're looking for a premium senior center designed exclusively for our cherished patients age 55 and above, Evara Gold would be the best choice.
