Know Before You Go

If you don't have insurance or plan to pay for your care yourself, you have the right to receive a Good Faith Estimate (GFE) of your expected out-of-pocket costs.

You can request a Good Faith Estimate when:

  • Scheduling an appointment at least three business days in advance
  • Asking about the cost of a service before scheduling an appointment

If you'd like a Good Faith Estimate, simply let us know when you schedule your visit or contact our Patient Access team.

If you don't have insurance or are concerned about the cost of care, you may qualify for Evara's Sliding Fee Discount Program.

Eligible patients can receive discounted medical, behavioral health, specialty, and dental services based on household income and family size.

You can apply when you establish care or during your appointment. Our team will walk you through the application, answer your questions, and help determine whether you qualify.

Good Faith Estimate

You may have the right to receive a "Good Faith Estimate" explaining how much your health care will cost. If you do not have insurance or you are choosing not to use insurance, you have the right to receive a Good Faith Estimate of the expected cost of your scheduled healthcare services. If your appointment is scheduled at least 3 business days in advance, Evara Health will provide a written Good Faith Estimate within the timeframe required by law. You may also request an estimate before scheduling care.
