Evara Health’s Food Rx Program
Because Healthy Food Is Medicine
At Evara Health, we know that eating well is a big part of staying healthy. That’s why we’ve teamed up with Feeding Tampa Bay to offer fresh fruits, vegetables, pantry staples, and even medically tailored meals to patients who need them most.
How It Works:
- Patients are screened for food insecurity using the USDA Hunger Vital Sign questions.
- If a patient is food insecure and has a diagnosis like diabetes, high blood pressure, or a high BMI, they may qualify for the program.
- Eligible patients receive a “food prescription” from their Evara Health provider.
- This prescription includes vouchers that can be used at any of our Food Rx Pharmacy locations.
- Our trained staff will help patients choose food options that support their specific health needs.
